  • General Rules for Oklahoma Close Up Student Participants
  • Student Medical Release Form
  • Program Terms and Conditions
  • Student Roommate Request Form
  • Registration Payment

The packet also includes:

  • School Participation Letter
  • Teacher Summary Statement
