In preparation for this year’s program, please download our 2012 Program Terms and Conditions to review with those students that will be attending our program.
2012 Program Terms and Conditions
In addition, please download the 2012 Student Medical Release Form. This form will need to be filled out and signed by the parent/guardian of the student attending the program. Teachers – please bring a completed form for each student attending the program with you on Tuesday, February 14, 2012. It will be collected by our registration team that afternoon. (No student can be admitted to the program without this form on file).
